It is a very simple document, but when presented to a veterinarian, it means the former owner is telling the vet to release everything to you. And you will need the medical records to avoid repeating treatment unnecessarily.
Chances are the owners won't remember the veterinarian's name, or they will make up something only for you to discover later that they never had an account with the vet they listed. In that case, shots will need repeated, and a heartworm test performed.
- Remind the former owners that if they really care about the dog, they will complete the form and help you have one less thing to deal with.
- If they won't sign the release, remind them that you have to charge a surrender fee to cover the cost of shots. Also remind them the local shelter will charge a drop off fee too (if applicable).
- Verify the information with the vet as soon as possible.
